Children's Court Forms
We are currently redesigning our forms. The new forms will give you options for filling them in and signing them. They are listed on this page as the following:
- Fillable Form with Digital Signature (PDF) - use this form to fill in on your computer and sign with a digital signature. More information is available from the Adobe website on how to set up a digital signature. Once completed the form can be emailed to the court. This option will not be available for all forms.
- Fillable Form (PDF) or Fillable Form (Word) - use this PDF or Word form to fill in on your computer, print, and sign with a pen. The form will then need to be scanned and emailed or sent to the court. Most fillable forms are PDF, if you need a Word version please contact the court.
- Print Only (PDF) - print this form out to fill in all sections and sign with a pen. The form will then need to be scanned and emailed or sent to the court.
To lodge your document, please email it to childrenscourt@justice.wa.gov.au or send to the court. The address is: 160 Pier Street, PERTH WA 6000.
Protection and Care documents may be lodged via email or through the eCourts Portal.
