Updated Children's Court Forms
Information Note
The Children's Court has updated the forms available on its website.
Forms have been changed, new forms have been added, and forms that are no longer needed have been removed. These changes are to better reflect current court practice.
Many forms have also been redesigned to meet accessibility requirements. You can find more information about accessibility on the website.
From 31 July 2026, all updated forms are available on the Children’s Court website. The forms are available as fillable and printable PDF documents.
A three-week transition period will apply. During this period, both the new and old versions of the forms will be accepted.
From 24 August 2026, the Children's Court Registry will no longer accept old versions of the forms. This includes listing certificates for criminal trials.
